Morcellation of apparent benign uterine myoma: assessing risk to benefit ratio
نویسندگان
چکیده
Specimen extraction is one of the most challenging steps during minimally invasive procedures. Theoretically, any surgical specimens have to be considered potentially malignant and therefore they have to be removed without contamination of the operative field [1]. However, solid and large masses (e.g., uterine myomas) cannot be removed easily, from the small abdominal incisions used for minimally invasive operations. Power morcellators, thanks to the fragmentation of these masses, allow their extraction from small laparoscopic incisions. Notwithstanding, the tissue morcellation may spread potential malignant surgical debris into the abdominal cavity, thus upstaging organ-confined malignancies to intraabdominal disease. Although the incidence of undiagnosed uterine malignancies is low, accumulating evidence supports that morcellation impacts on cancer patients’ prognosis [2].

Morcellation During Uterine Tissue Extraction AAGL Advancing Minimally Invasive Gynecology Worldwide I. Introduction: Minimally Invasive Surgery, Morcellation and the Scope of the Problem
The benefits of minimally invasive surgery (MIS) for treating a variety of gynecologic conditions are well documented [1-10]. Nearly half of the estimated 400,000 inpatient-based hysterectomies performed annually in the United States for benign indications employ these innovative techniques [11]. Thousands more women benefit from MIS in uterus-sparing procedures such as myomectomy. The ability ...
متن کاملParasitic Myomas Due to Laparoscopic Intra-abdominal Morcellation
Introduction: The use of electric tissue morcellators is crucial to allow the extraction of voluminous parts from a small laparoscopic incision, especially when the vagina is not opened. This report discusses how morcellation could be associated with the risk of intra-abdominal parasitic myomas developing. Cases Description: Three women were affected by intra-abdominal parasitic myomas several ...
متن کاملIntrapelvic dissemination of early low-grade endometrioid stromal sarcoma due to electronic morcellation
Endometrioid stromal sarcoma is a rare malignancy that originates from mesenchymal cells. It is classified into low-grade endometrioid stromal sarcoma (LGESS) and high-grade endometrioid stromal sarcoma. Ultrasonographic findings of LGESS resemble those of submucosal myomas, leading to the possible preoperative misdiagnosis of LGESS as uterine leiomyoma. Electronic morcellation during laparosco...
متن کاملComplex Laparoscopic Myomectomy with Severe Adhesions Performed with Proper Preventive Measures and Power Morcellation Provides a Safe Choice in Certain Infertility Cases
Laparoscopic myomectomy offers a real benefit to infertile patients with uterine fibroids and peritoneal adhesions. The procedure requires a skilled surgeon and laparoscopy technique to minimize adhesion formation and other proven benefits. Restrictions arise since this procedure requires power morcellation for fibroid tissue extraction. Two years ago, the Food and Drug Administration in the Un...
متن کاملP-188: Pregnancy Rates after Abdominal Myomectomy in Infertile Women
Background: Uterine myoma is the most common benign pathology during reproductive age. It has been suggested that the presence of uterine myoma may cause infertility and recurrent pregnancy loss. The purpose of this study was to determine the effect of abdominal myomectomy as a therapy on pregnancy rate in infertile women. Materials and Methods: We considered eligible for this study only the 82...
